My staff and I would like to welcome you to our practice. We appreciate your confidence in allowing us to administer your health care needs. We want you to know we are committed to providing you with the highest quality of oral health care in the most gentle, efficient and enthusiastic manner possible. In our office, we give each patient personal attention which allows you to have a positive dental experience. We are aware that some patients may be uneasy about dental care. We will make every effort to help relieve any apprehension you may have. In order for us to serve you, we must have good communication. We need to know your feelings and concerns regarding your oral health. It may be necessary for us to spend some time educating you about dental disease, treatment options and prevention. We want our patients to have an active role in their dental health. With this two way communication, we can strive to maintain your teeth for a lifetime. Since we want you to have a pleasant dental experience, let me explain your first dental visit. You will receive a thorough dental examination including your gums, teeth, facial muscles, bite relationship, oral cancer examination and jaw joint. A current full mouth series of digital x-rays are necessary to accomplish this thorough examination. If these x-rays have been taken previously at a dental office, please make arrangements to have them sent to our office or bring them with you. We value your time and, except for emergency situations, you can expect us to be on time for you. We will appreciate the same courtesy. Provided 48 hours advance notice is given, no charge will be made for rescheduling your appointment. This allows you time to be given to another patient. New patients are referred by our patients, and other dentists and physicians who recognize the value of high quality care and our continued commitment to excellence. Please let us know who referred you so we may thank them for their confidence in us. To better meet your needs, please complete the Medical History and Dental History forms and bring them to your appointment. We do request payment at the time of treatment. If you have any questions, please call. We look forward to meeting you. Sincerely,
